Mosaic Group’s Medications for Opioid Use Disorder (MOUD) Primary Care Practice Toolkit

The opioid epidemic continues to impact the nation, with overdose deaths at critically high levels. To help address this crisis, Mosaic Group developed the “Medications for Opioid Use Disorder Primary Care Practice Toolkit” with the support of the Maryland Department of Health.

About this Toolkit

This toolkit offers comprehensive guidance for healthcare providers, office staff, and patients, helping to streamline the integration of MOUD into primary care settings. By expanding access to this life-saving treatment, we aim to reduce overdose risk, enhance treatment retention, and improve the overall quality of care for individuals with opioid use disorder.

While the full toolkit will be available soon, you can download the individual tools listed below. Please select the tools based on your role in care—provider, office staff, or patient.

Note: This toolkit provides resources designed for specific audiences. Please ensure that only the tools intended for patient use are provided to patients.
