To help address this challenge, Mosaic Group developed the Screening, Brief Intervention, and Referral to Treatment (SBIRT) Primary Care Practice Toolkit to support integration of substance use screening and early intervention into primary care settings.

About this Toolkit

This toolkit offers practical, evidence-based guidance for primary care teams to identify, discuss, and address risky alcohol or substance use during routine visits. SBIRT promotes early intervention and strengthens the link between primary care and behavioral health. By embedding SBIRT into existing workflows, providers can enhance patient outcomes, improve care coordination, and promote whole-person health.
