Mosaic Group participated in the development of the “Improving Adolescent Health: Facilitating Change for Excellence in SBIRT” as part of the Facilitating Change for Excellence in SBIRT (FaCES) Practice Transformation Team. This tool was developed for the National Council for Mental Wellbeing and sponsored by the Conrad N. Hilton Foundation in July 2021. It outlines how to implement adolescent SBIRT in primary care facilities.

We are the National Council for Mental Wellbeing, a 501(c)(3) association and the unifying voice of organizations that deliver mental health and substance use services in America. 

The Conrad N. Hilton Foundation is a family foundation established in 1944 by the man who started Hilton Hotels. We provide funds to nonprofit organizations working to improve the lives of individuals living in poverty and experiencing disadvantage throughout the world.
